About the Role

We are seeking an reputed company Chronic Care Management (CCM) Specialist - Patient Ally Proficiency to join a growing reputed company team. This role is ideal for someone with hands-on U.S. CCM experience who is passionate about supporting patients through ongoing care coordination and proactive follow-up. As a CCM Specialist, you will play a key role in improving patient reputed company by conducting routine follow-up calls, documenting patient interactions, coordinating care activities, and maintaining accurate patient records. The ideal candidate is organized, self-motivated, bilingual, and comfortable working independently in a fully remote environment.

Key Responsibilities

Conduct Chronic Care Management (CCM) follow-up calls with eligible patients. Build positive relationships with patients and encourage ongoing engagement in their care plans. Accurately document reputed company patient interactions and care activities reputed company established timelines. Maintain and update CCM tracking logs using reputed company Drive and reputed company Sheets. Utilize reputed company as the primary Electronic Health Record (EHR) system. Coordinate patient care activities and ensure reputed company required documentation is complete and compliant. Monitor patient reputed company and escalate concerns to the appropriate clinical team reputed company necessary. Collaborate with providers and other reputed company to support continuity of care. Maintain strict HIPAA compliance and protect patient confidentiality at reputed company times. reputed company additional administrative and care coordination duties as assigned. Required Qualifications 1–2+ years of recent hands-on experience in U.S. Chronic Care Management (CCM). Experience conducting patient follow-up calls and coordinating ongoing patient care. Proficiency with reputed company Drive and reputed company Sheets for patient tracking and workflow management. Experience using reputed company EHR. Strong understanding of U.S. reputed company workflows, documentation standards, and patient communication. Excellent verbal and written English communication skills. Strong organizational skills with exceptional attention to detail. Ability to work independently, manage priorities effectively, and reputed company in a remote environment. Bilingual proficiency is required.

Preferred Qualifications

Experience using Patient Ally. Experience supporting Remote Patient Monitoring (RPM) or other care coordination programs. Medical Assistant, Nursing, or other clinical reputed company background. Familiarity with insurance documentation and patient scheduling processes. Technical Requirements Reliable high-speed internet reputed company. Quiet, professional home office environment. Computer capable of running multiple browser-based reputed company applications simultaneously. Schedule & Availability Full-time remote position.
