Assisting Hands Indianapolis West grew out of a desire to provide reliable, compassionate and personalized care for seniors and others needing non-medical assistance at home. As a Care Manager, the primary responsibilities are: Care Management and caregiver Oversight. Assisting Hands offers a team based, supportive work environment, opportunities for advancement, and competitive compensation.Responsibilities · Oversee and guide the care provided to clients by conducting regular home visits to ensure care plan is addressing needs of client · Conduct new client assessments and develop service plans · Review and update care plans by collaborating with Caregiving team · Independently manage weekly visit schedule and documentation · Develop and foster on-going relationships with referral sources and attend key marketing events · Provide coaching, support and assistance to Caregiving team · Attend weekly Care management meetings · Assist with Caregiver orientation and training · Participate in on call and holiday rotation · Provide back up client care occasionally if an urgent situation occurs Requirements· Three or more years in home care or a nursing facility or combination of facility and home care · Must have excellent communication skills with the ability to build relationships with caregiving staff, clients, families and referral sources · Must be able and willing to travel throughout the day to client locations. · Prior experience in supervising staff perfered · Must be have basic computer skills and proficient in Microsoft Office Suite Products
