Community Health Worker

At Aeetna Ohio, we're dedicated to empowering individuals to achieve and maintain better health outcomes and build stronger relationships with healthcare providers. We're seeking a compassionate and skilled Community Health Worker to join our team. This is a remote position offering a competitive salary of $28-$35 per hour.
What You'll Do:
- Conduct engaging, person-centered home visits to connect with Aeetna members.
- Educate individuals and families about health conditions, disease prevention, and self-management strategies.
- Build and maintain strong relationships with members, healthcare providers, and community resources.
- Identify and address social and economic barriers to healthcare access.
- Facilitate referrals to appropriate community resources and support services.
- Assist with medication adherence and promote healthy lifestyle choices.
- Collaborate with a team to enhance care coordination and improve health outcomes.
What You'll Need:
- Completed endorsed Community Health Worker (CHW) certification program (within the last year).
- Proficiency with computers, including navigation, Microsoft Office Suite (Word, Excel, SharePoint), and various online systems.
- Ability to travel up to 70% of the time to various office and home locations within Ohio using a personal vehicle.
- Valid driver's license, vehicle insurance, and reliable transportation.
- Flexibility to work in varying environments and adapt to changing circumstances.
- Excellent communication, interpersonal, and organizational skills.
- Experience in a community-based setting or with vulnerable populations is highly preferred.
- Knowledge of health insurance (Medicaid) and community resources is a plus.
