Remote Case Management Analyst

We're seeking a compassionate and detail-oriented Case Management Analyst to join our team at CVS Health. As a Case Management Analyst, you will play a vital role in delivering enhanced human-centric healthcare to our members. This is a full-time remote position, offering a competitive salary and a range of benefits.

Job Summary:

In this role, you will utilize critical thinking and judgment to collaborate and inform the case management process. You will facilitate appropriate healthcare outcomes for members by providing assistance with appointment scheduling, identifying and assisting with accessing benefits, and education for members through the use of care management tools and resources.

Key Responsibilities:

• Evaluate members' needs and eligibility, and recommend approaches to case resolution
• Identify high-risk factors and service needs that may impact member outcomes
• Coordinate and implement assigned care plan activities, and monitor care plan progress
• Enhance medical appropriateness and quality of care through consultation with case managers, supervisors, Medical Directors, and other health programs
• Utilize negotiation skills to secure appropriate options and services necessary to meet members' benefits and/or healthcare needs
• Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices

Requirements:

• 2+ years of experience in behavioral health or social services
• Ability to work 8:00a-5:00p CST
• 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook)
• Bachelor's degree or non-licensed master level clinician with either degree being in behavioral health or human services preferred
