Cigna is looking for a dynamic LPN/LVN Case Management Analyst to join our Medicare Clinical Operations team. As an LPN/LVN Case Management Analyst at Cigna, you will play a crucial role in ensuring the delivery of high-quality healthcare services to our Medicare beneficiaries.
Key Responsibilities:
Case Management: Work closely with the clinical team to assess and manage the healthcare needs of Medicare beneficiaries. Develop and implement care plans that align with Cigna quality standards.
Patient Advocacy: Serve as an advocate for our Medicare beneficiaries, ensuring they receive the appropriate care and support throughout their healthcare journey.
Data Analysis: Analyze healthcare data to identify trends, gaps in care, and opportunities for improvement. Utilize data-driven insights to make informed decisions.
Collaboration: Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to enhance the coordination of care and improve patient outcomes.
Compliance: Ensure that all case management activities adhere to regulatory and compliance standards, including Medicare guidelines.
Required Skills:
Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN): You must hold a valid LPN or LVN license in the state of practice.
Clinical Knowledge: Strong understanding of clinical assessments, healthcare delivery, and care coordination.
Data Analysis: Proficiency in data analysis and the ability to draw actionable insights from complex datasets.
Communication Skills: Excellent verbal and written communication skills to effectively interact with patients, healthcare professionals, and team members.
