About the position The Community Health Worker at UnitedHealthcare is responsible for assessing, planning, and implementing individualized care strategies for patients. This role focuses on managing care plans, addressing social determinants of health, and linking members to community resources and services. The position requires proactive engagement with members to improve their health outcomes and ensure they receive appropriate care while navigating the healthcare system. Responsibilities ? Create a positive experience and relationship with the member , ? Proactively engage the member to manage their own health and healthcare , ? Support the member to improve their wellbeing by staying out of the hospital and attending regular visits to their primary physician , ? Help the member engage with mental health and substance use treatment as needed , ? Provide member education on community resources and benefits , ? Conduct post-discharge activities from hospital facilities and support connection to social services , ? Partner with care teams including community providers and internal staff , ? Communicate health-related information to ensure quality coordinated care and services are provided , ? Advocate for patients and families to ensure their needs and choices are represented , ? Answer incoming phone calls from healthcare providers and identify the type of assistance needed , ? Resolve issues on the first call and navigate complex computer systems , ? Complete documentation to track provider issues and report overall trends , ? Monitor timely receipt, accuracy, and compliance of State assessments , ? Maintain adherence to State contract, CMS requirements, and performance metrics , ? Act as an advocate for membership and establish effective working relationships with providers , ? Work with hospitals, clinics, and the clinical team to manage requests for services from members and/or providers Requirements ? High School Diploma/GED (or higher) , ? 3+ years of experience working within the community health setting in a healthcare role , ? Intermediate level of computer proficiency including Microsoft Outlook, Teams, Excel, and Word , ? Ability to navigate through multiple web applications , ? Reside in the state of Texas , ? Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI) , ? Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection Nice-to-haves ? Bachelor's Degree in social work, licensed practical nurse (LPN), or CNA/HHA , ?
