Full Time RN Admission Nurse ($7,500 Sign on Bonus)

Responsibilities: • Performs admission and recertification assessments as delegated by the Clinical Director or designee. • Provides professional nursing care by utilizing all elements of nursing process. • Initiates the plan of care and makes necessary revisions as patient status and needs change. • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. • The Admissions Nurse submits Local Coverage Determinations as defined by Medicare and a Comprehensive nursing assessment to physicians, and obtains initial certification orders if not already obtained. • Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver. Qualifications and Requirements: • Graduate of an accredited school of nursing. • Current RN licensing, in good standing. • Valid drivers license, and auto insurance • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
