Remote Social Worker

Job Description: We are seeking a compassionate and experienced Social Worker to join our team remotely. In this role, you will provide support, coordination, and resources to individuals and families, helping them navigate complex healthcare and social service systems. The ideal candidate will have excellent communication skills, a strong sense of empathy, and the ability to work independently while maintaining a high level of professionalism. Key Responsibilities: • Assess clients' needs and develop individualized care plans to address their challenges and goals. • Coordinate access to social services, healthcare resources, and community programs to support clients’ well-being. • Provide counseling, emotional support, and advocacy to clients and their families. • Conduct virtual consultations, intake assessments, and follow-up meetings. • Document all client interactions and maintain accurate, up-to-date records in compliance with organizational policies and legal regulations. • Collaborate with healthcare providers, case managers, and other professionals to ensure comprehensive care. • Stay updated on social service programs, community resources, and changes in policies that may impact clients. Qualifications: • Bachelor’s or Master’s degree in Social Work (BSW or MSW) or a related field. • Active social work license (LCSW, LMSW, or equivalent) in the state of residence. • Previous experience in healthcare, social services, or case management preferred. • Strong interpersonal and communication skills with the ability to connect with clients virtually. • Proficiency in using technology and remote communication tools, including video conferencing platforms and electronic record systems. • Ability to manage a flexible schedule and work independently while meeting deadlines.
