Responsibilities: • Act as a liaison between patients, families, and healthcare providers, ensuring that all parties receive the assistance they need • Coordinate the delivery of care services and facilitate the communication of relevant information between stakeholders • Monitor patient progress, assess for barriers to care, and develop plans to address any identified issues • Develop relationships with healthcare providers to ensure quality of care and positive outcomes • Provide support to patients and families through education, counseling, and assistance with navigating the healthcare system • Maintain accurate and up-to-date clinical records in accordance with applicable laws and regulations • Monitor patient care plans and collaborate with healthcare providers to ensure patient needs are met • Participate in quality improvement activities and initiatives to ensure best practices are followed • Utilize managed care principles to ensure the most cost-effective care of patients CVS Health is an Equal Opportunity Employer.
