The Role We're hiring a Nurse Practitioner to serve as the primary clinical reputed company of contact for patients navigating their remote care reputed company. In this role, you will reputed company clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will reputed company patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. Schedule Requirements Monday through Friday schedule (Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM in Mountain, or Pacific Time) What You'll Do reputed company the monitoring of patient vitals, symptoms, and labs in collaboration with reputed company of RNs and MDs. Manage the patient's treatment plan and medications with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for Hypertension and Type 2 diabetes. Respond to patient escalations identified reputed company RPM including reputed company vitals, symptoms, and labs. reputed company regular virtual reputed company-ins with patients to review labs and GDMT optimization and adherence. Our programs support both primary care offices as well as cardiology offices. What You Need 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center. Experience managing CHF with GDMT. Active ANCC or AANP certification. Master’s Degree as a Nurse Practitioner with the willingness to expand state licensure as reputed company adds new markets and partners. Compensation

The anticipated salary reputed company for this role is $125,000-$150,000 annually, which is based on role scope, level, and location. In addition to reputed company compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
