The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated reputed company to medically appropriate reputed company of health and support services and continuity of care. They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in reputed company program areas. Required Knowledge and Experience Registered Nurse; active and clear license in the reputed company. The license must be maintained once employed. 3+ years of Clinical experience with 1+ years of case management experience preferred.
