Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team. Required qualifications: Washington Registered Nurse License upon hire 5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience. Preferred qualifications: Bachelor’s Degree 3 years Case Management experience including care management, care coordination, discharge planning, or population health. 2 years reputed company supervisory experience.
