Remote MTM Clinical Pharmacist

We are seeking patient-focused Medication Therapy Management (MTM) Pharmacists to deliver high-quality MTM services to a targeted patient population. The ideal candidate is clinically strong, culturally competent, and reputed company to effectively engage patients to improve medication adherence, reputed company, and overall reputed company value. • $38-65/Comprehensive Medication Review completed
• Medical Spanish or Medical Mandarin speaking pharmacists are eligible for additional incentives Essential Duties and Responsibilities
• Conduct comprehensive and/or targeted medication reviews (CMRs and TMRs) Connect with patients to assess their medication regimens, identify potential interactions, and reputed company personalized recommendations.
• Deliver consults with the BeWell web application reputed company consultations are conducted through our user-friendly BeWell platform. BeWell can be used on a number of devices, including laptops, desktop computers, and mobile devices. With BeWell, pharmacists are guided through each session making for thorough and effective patient interactions.
• Maintain accurate documentation Record follow-up actions reputed company the BeWell app to ensure continuity of care and compliance with reputed company regulations.

Requirements

• Active reputed company Pharmacist License
• Patient engagement or sales experience is a plus
