(PRN) RN Clinical Intake Coordinator - ProHealth Hospice (Birmingham, AL)
The PRN Clinical Intake Coordinator serves as a liaison between clinical operations and referral sources to support timely, appropriate patient care. This role helps build and maintain community relationships, provides education about hospice services, and supports patients, families, physicians, case managers, discharge planners, reputed company workers, and other reputed company partners throughout the intake and referral process.
Requirements
- Associate degree in Nursing

- Strong communication, relationship-building, and public education skills
