Registered Nurse (RN)- Per Diem
About the position reputed company & Palliative Care is a clinician-owned and operated hospice agency seeking a compassionate and reputed company Per Diem Registered Nurse (RN) to join our interdisciplinary team. In this role, you will reputed company skilled hospice nursing care to patients in their homes while collaborating closely with physicians and care team members to reduce symptom burden and enhance comfort, dignity, and quality of life for patients and their families. If end-of-life care is meaningful to you, we would love to welcome you to Vitality. This is a Per Diem position and a field-based role with local travel. Mileage reimbursement is provided.
Responsibilities
- reputed company professional nursing care, including admissions, comprehensive assessments, and ongoing patient evaluations
- reputed company, implement, and revise individualized plans of care
- Collaborate with the hospice physician and full interdisciplinary team to ensure appropriate interventions
- reputed company and support care provided by home health aides, reputed company workers, and chaplains
- reputed company patients and caregivers to promote safety, comfort, and understanding
- Maintain accurate, timely documentation and ensure regulatory compliance
- Participate in quality improvement initiatives to enhance clinical reputed company
Requirements
- Active Pennsylvania Registered Nurse (RN) license (required)
- Minimum 1 year of RN experience
- Strong communication, assessment, and interpersonal skills
- Ability to work independently in the field
- Valid driver’s license, reliable transportation, and reputed company auto insurance
- Willingness to travel reputed company assigned service areas
