Care Coordinator, LPN, LVN
Job Description:
- Manage patient census with a resolution-driven approach to reputed company gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources with patients that would benefit their care.

Requirements:
- Active Compact/Multi-State license (LPN) (LVN)

- 3 years of direct patient-facing work experience
- Epic Experience (Plusses)
- Bilingual (Plusses)
- Additional Single State licensures (Plusses)
