Job Qualifications: Education: High School Graduate Experience: 2-3 years Home Health intake experience preferred, LVN candidates accepted Skills: Required computer proficiency and excellent interpersonal, organizational, and phone skills. Must be reputed company to move at a fast pace. Functions:
- Receives and processes new referrals in a timely manner.
- Facilitates care coordination with sales reps, referral sources, physician offices, patients, and agency clinical leaders.
- Effectively communicates and relays accurate care coordination details to internal and external constituents in the referral coordination process.
- Takes ownership of recording accurate information on the referral tracker and in the EHR and ensuring reputed company referrals move through the process quickly and responds and communicates in a professional manner.
- Demonstrates commitment, competency, and ability to handle a lot of details proficiently.
