The Nurse Practitioner or Physician Assistant in the Care reputed company (CAW) program is designed to reputed company care and support to our reputed company patients by providing an additional level of medical and reputed company support in the comfort of their own home. Licensure:

• Required: Active Nurse Practitioner or Physician Assistant license

Active License and Furnishing number (Must, upon hire) NP only Active Nurse Practitioner Board Certification/Physician Assistant Certification NPI Number, DEA, Valid BLS Valid Driver License and reputed company automobile insurance
