Telehealth Physical Therapist / Occupational Therapist (Remote)
Ceres PostAcute Care - Location: Remote (Licensed in California) Setting: Skilled Nursing Facility (SNF) Employment Type: [Full-Time / Part-Time / PRN] Role Overview We are seeking innovative and tech-savvy Physical Therapists (PT) and Occupational Therapists (OT) to join our telehealth team. In this role, you will bridge the gap between traditional in-person care and modern digital health by providing expert rehabilitative services to residents in our Skilled Nursing Facility. You will work closely with onsite rehabilitation technicians and nursing staff to ensure residents receive high-quality, evidence-based care from the comfort of their rooms.
Key Responsibilities
- Virtual Evaluations: Conduct comprehensive remote assessments and evaluations to determine a resident's functional mobility, independence, and appropriateness for telehealth.
- Treatment Planning: Develop and implement individualized plans of care focused on restoration of function, pain reduction, and injury prevention.
- Session Delivery: Lead interactive, HIPAA-compliant video sessions to guide residents through therapeutic exercises and activities.
- Multidisciplinary Collaboration: Coordinate with onsite SNF staff, physicians, and families to align discharge goals and monitor resident progress.
- Resident & Caregiver Education: Provide remote training on home exercise programs (HEP), adaptive equipment, and safety strategies.
- Documentation: Maintain timely and accurate clinical documentation in the facility’s Electronic Medical Record (EMR) system (e.g., NetHealth, PointClickCare).
- Virtual Supervision: (For PTs/OTs) Supervise onsite therapy assistants or students as permitted by state licensure and facility policy.
Qualifications
- Licensure: Active, unrestricted license as a Physical Therapist or Occupational Therapist in [Insert State].
- Education: Graduate of an accredited PT or OT program (DPT, MPT, or MOT/OTD).
