Job Summary Responsibility for initiating phone contact with discharged patients from specified units, conducting query on patient experience and recovery, and providing needed follow up to patients and/or family members. Minimum Requirements Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred. Experience - One (1) year experience as a registered nurse.
