Responsibilities
Provide monthly calls and support to patients enrolled in the CCM program. , Utilize nursing skills to educate patients and their families. , Monitor patients' progress and work with physicians to ensure quality patient care.
Requirements
Active LPN license (compact) , Experience in geriatrics or primary care , Ability to sit and talk on the phone for long periods of time , System requirements: Windows i5 (or newer) processor, Windows Pro (not Home) Nice-to-haves
