What You'll Be Doing:

  • Engage patients telephonically to deliver clinical pharmacy services, including Medication Therapy Management (MTM) activities such as Comprehensive Medication Reviews (CMRs), Targeted Medication Reviews (TMRs), medication adherence counseling, medication reconciliation, and patient education.
  • Review and assess clinical interventions and documentation to ensure quality and consistency of care.
  • Evaluate, document, and maintain accurate patient records while preparing reports related to patient outcomes and program performance.
  • Collaborate with patients, providers, pharmacies, and other healthcare stakeholders to address patient needs and resolve issues in a timely manner, including transitions of care.
  • Develop and implement interventions aimed at optimizing medication use, improving adherence, and supporting quality initiatives.
  • Manage a high volume of patient and provider interactions while maintaining productivity, service standards, and documentation accuracy.
  • Meet established performance expectations related to outreach, engagement, quality, and clinical outcomes.
  • Adapt to changing priorities and support a variety of clinical initiatives based on business needs.
  • Identify opportunities to improve workflows and operational efficiency through collaboration and continuous process improvement.
  • Maintain effective communication and handoff practices to support continuity of care.
  • Provide guidance and support to team members and assist with onboarding and training activities as needed.

Who You Are and What You Bring:

  • Doctor of Pharmacy (PharmD) from an accredited college or university.
  • Active Utah Pharmacist license in good standing; additional state licenses are preferred.
  • Knowledge of CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C and Part D medication measures.
  • Strong understanding of chronic disease management and care for patients with complex medication regimens.
  • Comprehensive knowledge of prescription and over-the-counter medications, including indications, dosing, contraindications, and adverse effects.
  • Ability to evaluate medication appropriateness while considering patient-specific factors and clinical considerations.
  • Experience engaging patients to promote adherence using motivational interviewing and counseling techniques.
  • Excellent communication and interpersonal skills with the ability to build rapport and navigate sensitive conversations.
  • Experience working with diverse patient populations and demonstrating cultural sensitivity.
  • Strong attention to detail, accountability, and commitment to documentation quality.
  • Ability to work independently, prioritize competing responsibilities, and meet deadlines in a fast-paced environment.
  • Collaborative mindset with openness to feedback, coaching, and continuous learning.
  • Proficiency with technology and the ability to navigate multiple systems efficiently in a phone-based environment.
  • Willingness to obtain additional pharmacist licenses, as needed.
  • Availability to work 40 hours per week within operating hours of Monday-Friday, 6:00 AM-6:00 PM PT, including up to two rotating Saturdays per month.

Preferred Qualifications:

  • Preference for CA, NY, UT, VA, or TN licensure.
  • Bilingual
  • Experience in ambulatory care, chronic disease management, managed care, PBM, health plan, or ACO settings.
  • Previous MTM experience utilizing a variety of MTM platforms.
  • Experience supporting geriatric populations or long-term care settings.
  • Background in hospital, inpatient, or emergency department pharmacy.
  • Experience in high-volume, metrics-driven healthcare environments.
  • Prior remote or virtual healthcare experience.
  • Board certification and/or completion of a PGY1 or PGY2 residency program.

Remote Work Requirements:

  • Dedicated private workspace that maintains patient confidentiality.
  • Reliable high-speed internet capable of supporting video and voice communications without interruption.
  • Ability to attend occasional in-person meetings, including onboarding and team meetings, as required.

Requirements:

  • Ability to successfully pass a background check.
  • Must reside in and be authorized to work in the United States.
