Job Summary
We are seeking an Experienced Licensed Social Worker to join our team of dedicated professionals who share our passion for delivering exceptional care and support to our members. As a remote worker, you will have the flexibility to work from anywhere in the U.S. and enjoy a dynamic and collaborative work environment that values diversity, inclusion, and belonging.
Responsibilities
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Empower members to make positive lifestyle and medical choices through education and support
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to provide support and advocacy
- Develop and implement personalized plans to meet the unique needs of each member
Requirements
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work
