Job Summary
As a Remote Social Worker, you will play a vital role in our medical case management and community resource coordination team. You will work closely with our registered nurse case managers to assess, plan, and advocate for our members' benefit plans and health needs. Your expertise will be essential in researching available options and resources for members with financial, transportation, safety, and housing needs. You will empower our members to make positive lifestyle and medical choices, and work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
- Collaborate with registered nurse case managers to assess, plan, and advocate for members' benefit plans and health needs - Research available options and resources for members with financial, transportation, safety, and housing needs - Empower members to make positive lifestyle and medical choices - Work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health - Utilize a collaborative process of assessment, planning, and advocacy to meet members' benefit plans and health needs - Communicate effectively with members, families, and healthcare providers through telephonic or electronic exchange
Requirements
- Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence - 2+ years of experience in medical social work or case management - Masters Degree in Social Work

- Experience working with diverse populations, including children, adults, and families - Strong communication and interpersonal skills - Ability to work independently and collaboratively as part of a team - Familiarity with community resources and services - Experience with electronic health records and case management software

Compensation and Benefits
