About the Opportunity

We're seeking a highly motivated and detail-oriented Operations Support Associate to join our team. As a key member of our Care Operations team, you'll play a critical role in ensuring that our patients receive the best possible care. You'll be responsible for completing backend operational support processes, ensuring patient ECM and CHW program authorization and continued enrollment, and creating a foundation of process efficiency and improvement.

What You'll Do

As an Operations Support Associate, you'll have the opportunity to make a meaningful impact on our operations and contribute to the success of our team. Your key responsibilities will include:

• Own key Ops Support initiatives: Take ownership of critical operational support processes and initiatives, ensuring that they're completed efficiently and effectively.
• Proactively monitor and report on key operational support processes and metrics: Stay on top of key performance indicators and report on operational support processes to ensure that we're meeting our goals.
• Support a team of documentation analysts: Troubleshoot and monitor project progress, providing support and guidance as needed.
• Complete and monitor ECM and CHW program authorizations and annual reauthorizations: Ensure that patient enrollment and continuation of care are maintained, and that program authorizations are up-to-date.
• Work with partnered health plans to understand program denials and flag any enrollment issues: Collaborate with health plans to identify and resolve enrollment issues, ensuring that patients receive the care they need.
• Provide backend support for transitions of care and referrals: Ensure that closed-loop follow-up is completed from the care team, and that patients receive seamless care transitions.
• Provide clear, timely communication to our care team when there is an enrollment change or status update: Keep our care team informed of any changes or updates, ensuring that they have the information they need to provide the best possible care.
• Support Care Operations leaders in owning and optimizing key standard processes: Work with our leaders to identify areas for improvement and propose solutions to optimize our processes.

What You'll Need

To succeed in this role, you'll need:

• 1+ years of backend operations experience in a fast-paced healthcare environment providing administrative and operational support: You'll have a strong understanding of operational support processes and a proven track record of delivering results in a fast-paced environment.
• Confidence to own your role and propose solutions to be successful: You'll be a self-starter who's not afraid to take ownership of your work and propose solutions to drive success.
• A results-driven attitude with a proven track record of exceeding performance indicators: You'll be focused on delivering results and exceeding performance indicators, with a strong commitment to achieving our goals.
