Experienced Licensed Social Worker – Remote Medical Social Work and Case Management

Job Summary
As a Licensed Social Worker in this remote role, you'll work with a registered nurse case manager to assess, plan, and advocate for members with financial, transportation, safety, and housing needs. You'll research available options and resources to empower members to make positive lifestyle and medical choices. This role requires an LSW/MSW with an unrestricted active license, and you'll work with a team of clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Empower members to make positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members
- Collaborate with other healthcare professionals to ensure comprehensive care
Requirements
* Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in state of residence
- 2+ years experience in medical social work or case management
- Masters Degree in Social Work
