Urology Telehealth Nurse Practitioner
We are seeking an energetic, compassionate, skilled Nurse Practitioner or Physician Assistant with strong Urology experience to join Post Acute Specialists in Florida. We are a multispecialty provider telemedicine group specializing in post-discharge care to help bridge the gap from hospital to home. Our team conducts virtual telehealth visits with senior patients in their homes, assisted living communities, or skilled nursing facilities within 72 hours of hospital discharge. This provider will have a high level of autonomy in managing urological conditions in parallel with our supervising physician and play a vital role in improving patients' quality of life. Job Responsibilities include but are not limited to:
- Patient assessments, treatment plans and follow-up visits.
- Recognize scenarios which would require escalation of care.
- Order and evaluate test results and collaborate with supervising physician and other healthcare providers.
- Asses and problem-solve following medical guidelines and protocols.
- Develop patient care pathways, protocols and guidelines for managing specific problems with physician and other health care team members.
- Serve as a knowledge resource and act as a role model in medical and nursing practices.
ESSENTIAL ABILITIES:
- Detailed knowledge and experience in diagnosing, treating and managing various conditions affecting the urinary tract including urinary tract infections, urinary incontinence, overactive bladder, kidney stones and prostate issues
- Ability to provide virtual telehealth visits to seniors who are in their homes or in post acute facilities in Florida
- Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
- Comfortable working independently and in collaboration with other medical team members in utilizing approved protocols, procedures, and expert consultation for our cardiovascular patients.
QUALIFICATIONS
- Master's degree in Nursing Licensure, Certification, and Registration or a Master of Physician Assistant Studies License
- Current, valid Florida Advanced Practitioner Licensure
- Current BLS required
- Urology-focused clinical experience (3+ years required)
- CUNP Certification highly desired
- Geriatrics and telehealth experience are highly preferred.
