Job Summary

We're seeking an experienced Licensed Social Worker to join our remote medical case management team. As a Licensed Social Worker, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population, spanning the entire lifecycle, and all member contact will be done telephonically or via electronic exchange.

Key Responsibilities

* Research and identify available options and resources for members with financial, transportation, safety, and housing needs

• Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs
• Empower members to make positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Develop and implement individualized plans to meet members' needs
• Maintain accurate and up-to-date records of member interactions and progress

Essential Qualifications

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
• Ability to maintain confidentiality and handle sensitive information

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and older adults

• Knowledge of community resources and services
• Experience with electronic medical records and other health care software
• Strong analytical and problem-solving skills
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities

arenaflex is a remote work environment, and this role will require you to work from home. You'll have the flexibility to work from anywhere in the U.S. and will be expected to work Monday-Friday, 8am-4:30pm Pacific Standard Time. • Base salary: $46,597.20 - $101,228.40
