Position Overview: The Value-based Pay and Population Health Program Manager oversees and optimizes OCHIN’s systems, processes, initiatives, and tools to support the success of our members. This role leads strategic programs, focusing on equitable clinical, financial, and operational outcomes for OCHIN members. The Business Partner works closely with management to define program goals and ensures alignment across teams to drive successful outcomes. Responsibilities include providing consultation and technical assistance, managing grant compliance, fostering positive funder relationships, and acting as the program’s voice to ensure stakeholder communication and process alignment. This role plays a critical part in the execution of OCHIN's strategic goals and the achievement of desired outcomes for all stakeholders. Essential Functions:

  • Program Leadership & Execution: Lead the full lifecycle of strategic programs—from scoping and planning to execution and optimization—ensuring alignment with OCHIN’s mission and organizational goals.
  • Stakeholder Engagement & Communication: Build and maintain strong relationships with internal teams, executive leadership, and external partners. Serve as the primary point of contact for program inquiries and regularly communicate progress, outcomes, and roadmaps.
  • Project & Program Governance: Apply advanced project management techniques to prioritize tasks, manage resources, mitigate risks, and uphold program governance, quality standards, and customer satisfaction.
  • Data-Driven Strategy & Continuous Improvement: Use data and analytics to guide program direction, improve deliverables, and enhance member experience. Identify opportunities to increase adoption and optimize tools, technologies, and processes.
  • Organizational Alignment & Brand Stewardship: Stay informed on industry trends, regulatory requirements, and internal services. Promote a member-centric culture and uphold OCHIN’s brand integrity through consistent use of tools, documentation, and knowledge-sharing practices.
  • Other duties as assigned.

Requirements

  • Bachelor's degree required, master's degree preferred
  • Minimum 5 years' experience in program/project management required
  • Minimum 3 years of Epic Ambulatory and/or Healthy Planet experience strongly preferred (proficiency in both applications a plus)
  • Healthcare Payer experience preferred
  • PMP a plus
  • Proven leadership and team management capabilities, with the ability to motivate and guide cross-functional teams.
  • Excellent communication and interpersonal skills to effectively engage with diverse stakeholders.
  • Strong analytical and problem-solving skills to support strategic decision-making.
  • Experience in budgeting and financial management to ensure program sustainability and resource alignment.
  • Demonstrated success in stakeholder engagement and relationship management across internal and external partners.
