Occupational Therapist (OTR/L) – In-Home Rehabilitation Specialist
About the position This is a field-based opportunity for an Occupational Therapist (OTR/L) who values autonomy, efficiency, and meaningful one-on-one care. Responsibilities
- Conduct in-home OT evaluations focused on safety, function, and independence.
- Develop treatment plans that drive measurable ADL and mobility outcomes.
- Provide hands-on therapy to improve strength, coordination, and daily function.
- Perform home safety assessments and recommend adaptive equipment.
- Manage territory efficiently to maximize time and impact.
- Complete accurate EMR documentation and collaborate with the care team.
Requirements
- Active Minnesota OTR/L license.
- Strong self-management and ability to work independently.
- Organized, efficient, and outcome-driven mindset.
- Reliable transportation for field-based visits.
- Passion for helping patients regain independence in their home.
