Provide individual and group therapy sessions to address mental health issues and promote overall well-being. Collaborate with interdisciplinary teams to coordinate care and ensure a holistic approach to treatment. Offer guidance and support to patients and their families to help them navigate through difficult situations and access necessary resources. Maintain accurate and up-to-date documentation of patient progress and treatment plans. Stay informed about current social work practices and continuously seek opportunities for professional development. Adhere to ethical standards and legal requirements in all aspects of the job. Advocate for patients' rights and work towards removing barriers to their access to healthcare. Utilize evidence-based practices to promote the best possible outcomes for patients. Participate in team meetings and case conferences to discuss patient progress and collaborate on treatment plans. Educate patients and their families on coping skills and strategies to manage their mental health. Provide crisis intervention and support to patients in times of acute need. Serve as a liaison between patients, their families, and community resources to ensure continuity of care. Demonstrate empathy, compassion, and cultural sensitivity in all interactions with patients and their families.
