Transition Requisition - Campus Health Advanced Practice Providers

As part of the Campus Health Advanced Practice Provider transition, we will be creating a new employment record for you. This on-line process is not an application for employment, but a way for us to collect information needed to set up your UNC Health employee record. 

During this process, you will: 

• Review and acknowledge UNC Health’s employment statement 

• Provide general demographic information 

• Share your education and work history 

  To help make this step quicker and easier, please upload a current resume—it will automatically fill in much of your work history for you. Be sure to include your home address & phone number on your resume and double check that the Work Experience and Education information populates properly onto your Employment Profile.

You can complete this process from any computer with internet access, and it should take about 10-15 minutes. The information you provide is essential for us to create your employee record and ensure a smooth transition.

REMINDER: Please use a personal email (non UNC email) address to complete the process. Include all relevant work experience and the month & year start date/end date for each position.
