Job Summary
We're seeking a highly skilled Licensed Social Worker to join our remote team. As a medical case manager and advocate, you'll work closely with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, using telephonic and electronic exchanges to empower members to make positive lifestyle and medical choices.
Key Responsibilities
* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Work with a diverse population across the entire lifecycle, using telephonic and electronic exchanges to empower members to make positive lifestyle and medical choices
- Partner with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Empower members to make informed decisions about their healthcare and well-being
- Develop and implement personalized care plans to meet members' unique needs
- Maintain accurate and up-to-date records of member interactions and care plans
Essential Qualifications
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
- 2+ years of experience in medical social work or case management
- Master's degree in Social Work or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Ability to maintain confidentiality and handle sensitive information

* The typical pay range for this role is $46,597.20 - $101,228.40
