Arizona Home Care is seeking a Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the homes of individuals throughout Phoenix and the surrounding Valley.

This role is ideal for a confident RN who enjoys independent practice, strong care coordination, and hands-on clinical nursing in a home health and home infusion setting.

Evening Schedule: 3 PM – 11 PM

Full-time, field-based role

Home Health RN Job Summary

The Home Health Registered Nurse (RN) is responsible for the assessment, planning, implementation, and overall case management of patients receiving General Home Care and Patient Care Infusion services. The RN provides skilled nursing care in patients’ homes, coordinates care with physicians and interdisciplinary teams, educates patients and caregivers, and ensures compliance with all applicable regulatory and accreditation standards.

Home Health RN Job Responsibilities

Assessment & Care Planning:

  • Conduct comprehensive initial and ongoing assessments of patients’ clinical, social, environmental, and safety needs
  • Identify home safety risks and educate patients and families to minimize care-related hazards
  • Develop, initiate, and revise Plans of Care in collaboration with physicians
  • Evaluate patient and caregiver ability to manage care independently

Skilled Nursing & Infusion Care:

  • Provide skilled nursing services within RN scope and agency policy, including:
    • Wound care (including wound vacs)
    • Infusion therapy and IV medication administration
    • Venipuncture, IV catheter insertion, line care, and lab draws
  • Monitor infusion therapies, adjust flow rates as indicated, and observe patient response
  • Adhere strictly to infection control, safety standards, and universal precautions

Case Management & Coordination:

  • Assume overall responsibility for assigned patient cases
  • Coordinate services with pharmacy, supervisors, LPNs, CNAs, and interdisciplinary team members
  • Supervise LPNs and CNAs as appropriate to the plan of care
  • Communicate changes in patient condition promptly to physicians and care team
  • Plan and coordinate patient discharge from services

Education, Documentation & Compliance:

  • Educate patients and caregivers on diagnoses, treatments, medication regimens, and care techniques
  • Document patient competency and compliance with care plans
  • Complete accurate, timely documentation in accordance with regulatory and agency policies
  • Participate in case conferences, in-services, and required education annually

Requirements

  • 2+ years of home health nursing experience
  • Active Arizona RN license
  • Graduate of an accredited school of nursing
  • Current CPR/BLS/First Aid certifications
  • Valid Arizona Level One Fingerprint Clearance Card
  • Valid driver’s license, reliable transportation & proof of auto insurance
  • Valid TB test results
  • Experience with infusion therapy and wound care
  • Familiarity with Medicare OASIS documentation
  • Proficiency with EMRs and point-of-care documentation
  • Ability to meet physical demands of home health nursing
  • Strong clinical judgment, organizational skills, and communication abilities
