Location Based in: supporting high-risk patients remotely.

Your Role as a Virtual Care Nurse (LPN)

Support the Complex Care Team by providing virtual care to patients at high risk for hospitalization.

Conduct virtual visits to assess patient needs and provide education on managing medical conditions.

Monitor telehealth systems and track patient data to identify hospitalization risks.

Collaborate with the care team to facilitate patient care across the continuum of services.

Attend rehospitalization-related meetings, conferences, and symposiums to enhance patient care strategies.

Assist in developing and implementing best practices for virtual care.

Manage virtual care and telehealth systems through assessments, education, and data tracking.

Qualifications

Active and unencumbered Florida Licensed Practical Nurse (LPN) license (required).

Minimum of two (2) years of experience as a home health clinician.

Strong clinical background with experience in triage and hospitalization prevention (required).

Proficiency in Microsoft Office (preferred).
