*This is a full time position that will service the North Phoenix area (Cave Creek, Carefree, North Scottsdale) OR the West Phoenix area (Glendale, Peoria).

*$80000 - $90000 per year (depending on experience), plus travel expenses.

What You’ll Do

As a Home Health RN, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:

• Build meaningful relationships with patients and families
• Create personalized care plans that truly make an impact
• Coordinate care as a trusted Case Manager
• Educate and empower patients and caregivers with confidence
• Collaborate with an interdisciplinary team that shares your commitment to excellence

What We’re Looking For

• Current RN license in the State of Arizona
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and make a real difference
