Medical Director

Work from home Full-time role Hiring

Medical Director Location: Flexible (within Peregrine service regions) Employment Type: Full-Time (3 Administrative Days / 2 Clinical Days) Department: Clinical Leadership Reports To: Chief Executive Officer

About Peregrine

Peregrine is a fast-growing behavioral health services and technology platform that partners with Federally Qualified Health Centers (FQHCs) and Community Health Centers (CHCs) to expand access to high-quality mental health and substance use disorder care. We combine technology, clinical operations, and mission-driven partnerships to bring behavioral health services to underserved communities across multiple states.

Position Summary

The Medical Director will serve as a key member of Peregrine’s leadership team, responsible for clinical oversight, medical governance, and partnership with operational and product teams to drive clinical excellence across the organization. This leader will split their time between administrative leadership and direct patient care .

The ideal candidate brings a deep understanding of community health, value-based care, and MAT programs, with a passion for expanding access to care in underserved communities. This is both a clinical leadership and a strategic growth role: the Medical Director will help shape Peregrine’s revenue-generating clinical programs (e.g., CoCM), standardize clinical processes across markets, and represent Peregrine externally as a thought leader.

Key Responsibilities

Clinical Leadership and Governance

  • Serve as the Professional Corporation (PC) Owner for Peregrine’s clinical practice entities.
  • Establish clear clinical protocols, decision frameworks, and escalation pathways for scenarios involving partners, providers, and patients, including crisis protocols and legal escalations (Board of Nursing, subpoenas, partner executives, etc.).
  • Partner with Market Operations leadership to ensure operational teams understand and execute basic clinical protocols, especially around crisis and risk situations.
  • Lead the development and enforcement of standardized appointment templates, scheduling practices, and clinical workflows to ensure consistency, efficiency, and quality of care.
  • Design and implement peer review, clinical quality initiatives, and compliance processes to meet regulatory standards and support program growth.

Strategic Initiatives and Program Development

  • Partner with the CEO and leadership team to evaluate new care models, scope of services, and regulatory structures to support sustainable growth.
  • Develop scalable frameworks for supervising physician coverage across markets to reduce cost structure without compromising quality or compliance.
  • Represent Peregrine in external presentations, partner webinars, and conferences, traveling to build relationships with partner executives and clinical teams.
  • Gather clinical protocols and processes from partner organizations, then synthesize and establish Peregrine Standard Protocols to be applied consistently across markets.

Administrative and Strategic Leadership

  • Serve as a bridge between clinical and operational teams, ensuring alignment between frontline care delivery and organizational strategy.
  • Participate as a member of Peregrine’s leadership team, contributing to strategic planning, program design, and growth initiatives.
  • Collaborate with product and data teams to evaluate clinical outcomes, identify gaps, and inform continuous improvement efforts.
  • Support recruitment, onboarding, and retention of high-performing clinical staff, including setting clear expectations for scheduling, documentation, and time management best practices.

Clinical Care

  • Provide direct patient care with a focus on behavioral health and MAT programs.
  • Backfill clinical coverage as needed during provider absences or temporary staffing gaps.
  • Model high-quality, patient-centered care consistent with Peregrine’s mission and standards.

Qualifications

  • MD or DO with active, unrestricted medical license.
  • Board certification in Psychiatry, Family Medicine, Internal Medicine, or related specialty with behavioral health experience.
  • Minimum 5 years of clinical experience, preferably in community health, FQHC, or MAT settings.
  • Demonstrated experience building or scaling clinical programs (e.g., CoCM, value-based models, MAT).
  • Prior leadership experience within a healthcare organization strongly preferred.
  • Eligible to own or serve as physician of record for a Professional Corporation in relevant states.
  • Strong understanding of clinical compliance, telehealth regulations, and operational integration.
  • Excellent communication and collaboration skills, with the ability to build trust across clinical and non-clinical teams.
  • Willingness to travel regularly for presentations, partner engagement, and leadership visibility.

Additional Considerations

  • This is a full-time leadership role; outside employment or affiliations must comply with Peregrine’s inventions and exclusivity agreements.
  • The Medical Director should be comfortable working at the intersection of clinical practice, operations, compliance, and business growth.
  • The role is viewed as both a clinical leadership investment and a growth and marketing lever, central to Peregrine’s ability to scale sustainably.
