Job Summary

We're seeking an experienced licensed social worker to join our remote medical social work and case management team. As a social worker at arenaflex, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, using telephonic and electronic communication to empower members to make positive lifestyle and medical choices.

Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs

• Research available options and resources for members with financial, transportation, safety, and housing needs
• Work with a diverse population across the entire lifecycle, using telephonic and electronic communication to empower members to make positive lifestyle and medical choices
• Partner with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Empower members to make informed decisions about their healthcare and well-being
• Develop and implement individualized plans to address members' needs and goals
• Maintain accurate and up-to-date records of member interactions and progress

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence

• 2+ years of experience in medical social work or case management
• Masters degree in social work or a related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
• Ability to maintain confidentiality and handle sensitive information

Compensation and Benefits

The typical pay range for this role is $46,597.20 - $101,228.40. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors. This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
