Job Summary

As a remote social worker at blithequark, you will be responsible for partnering with a registered nurse case manager to provide comprehensive support and resources to our members. Your primary responsibilities will include:

• Conducting assessments and developing plans to address members' financial, transportation, safety, and housing needs
• Researching and connecting members with community resources and services that support their medical, financial, and behavioral health
• Empowering members to make informed decisions about their health and well-being through education and advocacy
• Collaborating with other clinical staff to ensure seamless care coordination and support
• Utilizing telephonic and electronic communication to engage with members and provide support

Requirements

To be successful in this role, you will need to possess the following qualifications and skills:

• Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
• 2+ years of experience in medical social work or case management
• Master's degree in social work or a related field
• Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations
• Strong assessment and planning skills, with the ability to develop and implement effective plans to address complex needs
• Ability to work independently and collaboratively as part of a team
• Proficiency in electronic communication and documentation systems
• Strong problem-solving and critical thinking skills, with the ability to navigate complex systems and resources
