A Pharmacist is responsible for monitoring, evaluating and making recommendations to assure the appropriate and effective use of pharmaceuticals for patient care. The ROE pharmacist shall use the medication process to assess, plan, intervene, when necessary, implement and evaluate patient care in compliance with client hospital and ROE policies, protocols and procedures.

A Pharmacist who excels in this role:
Assists in the orientation and training of the ROE pharmacistsFollows all policies and procedures of the ROE unitProvide patient care activities to ensure safe and effective drug therapyBe of service to patients, physicians, nurses and other pharmacistsMaintain and improve job knowledge, skills and competencePerform duties in accordance with specific standards of care as evidenced by completion of competency skills checklistArticulate daily activities that support continuous quality improvement of patient carePrepare written or oral report at change of shift to communicate incomplete assignments and potential patient safety concerns to monitor in order to ensure continuity of medication therapy if requiredDemonstrate knowledge and mastery of required job functions, internal communications and operating systemsAccurately enter orders in the computer in a timely mannerSupport and assist in meeting overall goals both operationally for order entry in addition to the clinical programs, including, but not limited to therapeutic interchanges set forth by LifePointFollows facility procedures when non-formulary drugs, target drugs, restricted drugs and investigational drugs are orderedDemonstrate proficiency in computer order entry and related computer functionsScreen for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entryAssure facility queues are addressed in a timely mannerInvestigate, report, and document adverse drug events and medication incidentsFacilitate dissemination of drug information in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes when consultedReview and interpret all physician orders received, using the patient profileMonitor for incompatibilities, concentration and rate on intravenous drugsParticipate in the maintenance of continuous quality improvement programDocument clinical interventions and follow-up when indicatedAssess orders for age-specific appropriateness from neonatal through geriatricPerform other duties as deemed necessary by the ROE Director or Corporate Director of Pharmacy Services, in accordance with established policies and requirements of accrediting bodiesPractice and adhere to “Code of Conduct” philosophy and “Mission and Value Statement”What we’re looking for
Applicants should have a Bachelor’s degree in Pharmacy or Doctoral degree preferred from an accredited college of pharmacy with three (3) years hospital experience required
Pharmacy Licenses in the state of North Carolina and VirginiaMeditech or HMS operating system experience preferredWhy join us
