Job Summary
As a LPN - Home Health with Enhabit, you'll play a vital role in providing nursing care to patients in their homes under the direction of Registered Nurses (RNs) and in compliance with physicians' orders. Key Responsibilities
- Visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders

- Collaborate with RNs and other healthcare professionals to develop and implement patient care plans
- Maintain accurate and up-to-date patient records

Qualifications
Essential Qualifications
- Current licensure as a Licensed Practical Nurse (LPN) in the state of employment
- Valid state driver's license
- Automobile liability insurance
- Dependable transportation kept in good working condition
- Ability to drive an automobile in all types of weather conditions
