Job Overview: Hospice RN Triage 7on/7off (Remote)

We're seeking an experienced and compassionate Hospice RN Triage Nurse to join our team on a remote basis, working a 7 days on/7 days off schedule. As a Hospice RN Triage Nurse, you'll play a critical role in providing quality phone triage to our patients during off-hours and weekends, administering nursing triage for patients of all ages in their place of residence, and coordinating care with the on-call RN if a visit is needed.

Key Responsibilities:

• Provide expert phone triage to patients and families during off-hours and weekends, assessing their needs and developing effective care plans.
• Administer nursing triage for patients of all ages in their place of residence, utilizing your knowledge and expertise to address their unique needs.
• Coordinate care with the on-call RN if a visit is needed, ensuring seamless transitions and high-quality care.
• Maintain accurate and detailed records of patient interactions, ensuring compliance with regulatory requirements and organizational standards.
• Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
• Utilize your knowledge of palliation and end-of-life care to provide expert guidance and support to patients and families.

Essential Qualifications:

• Active RN license in the state of TX, OK, or MO.
• Minimum of 2 years of hospice experience, with a strong background in end-of-life care.
• Ability to work independently and make sound decisions, with excellent critical thinking and problem-solving skills.
• Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, and healthcare professionals.
• Strong attention to detail and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
• Flexibility to work on-call shifts, including weekends and holidays, with a reliable transportation arrangement and valid auto liability insurance and driver's license.

Preferred Qualifications:

• Specialized skills in palliation and end-of-life care, with a deep understanding of the unique needs of hospice patients and their families.
• Experience working in a remote or telehealth environment, with a strong ability to adapt to new technologies and workflows.
• Certifications or advanced training in hospice and palliative care, such as CHPLN or CHPN.
